Eligibility for Services and Supports

The Home and Community Based Services (HCBS) Intellectual / Developmental Disability (I/DD) Waiver is for Kansas residents age 5 and older. Eligibility determination is the first step to accessing services or getting placed on the waiting list for the HCBS I/DD Waiver in Shawnee County. See the Kansas eligibility requirements for this waiver below. Currently there is a waiting list for the funding of these services.

I'm on the waiting list. What now?

Once you've been determined eligible through the CDDO, completed a Functional Assessment, and are eligible for the I/DD waiver, you will be placed on the statewide waiting list. If you would like to confirm the date you were added to the waiting list, you may contact the CDDO. 

While you are waiting, it is important to maintain current contact information with the CDDO. Please notify us at (785) 232-5083 if your phone number or address changes. The CDDO will make contact with you annually by mail. Please sign and return any forms that are mailed to you. 

If you are Medicaid eligible, you may ask to be connected with a Targeted Case Manager (TCM). Currently there is a waiting list in Shawnee County for TCM services. Please contact the CDDO to be added to the TCM waiting list.
