Choosing or Changing Service Providers
If you are choosing a provider for the first time or are interested in making a change to your current provider(s), the CDDO will provide you with Options Counseling. To begin this process, you may contact your Targeted Case Manager or the CDDO. The CDDO will impartially provide you with information on the types and availability of services in Shawnee County, as well as provide you with a current listing of providers accepting new persons into services. You have the right to change service providers at any time.
Functional Assessment
The functional assessment is completed to determine eligibility for HCBS I/DD waiver. This assessment collects information about your demographics, daily living skills, medical conditions, and behaviors. Initial assessments are completed for individuals determined eligible for I/DD services. Annual assessments are completed for individuals receiving services to capture any changes that may have occurred. Your TCM will contact you to schedule your annual assessment. As of July 1, 2025, the MFEI (Medicaid Functional Eligibility Instrument) tool is being used to complete the functional assessment.
