
Functional Assessment

When should the Assessment be scheduled?
Assessments must be scheduled between no more than 30 days and no later than 7 days prior to the date of the last assessment. Case Managers must coordinate and confirm the assessment date, time, and location at least 6 weeks prior to the due date to avoid scheduling conflicts and to account for unforeseen circumstances.
Example: Last assessment date is 1/18/24 – get the assessment on the calendar no later than 12/07/24 (6 week scheduling due date). The assessment should be scheduled between 12/19/24 (earliest) and 1/11/25 (latest – 358th date).
It is the responsibility of the Case Manager to track the due date of the assessment and schedule accordingly.
If an assessment has been scheduled, and needs to be re-scheduled, or needs to be scheduled at a time outside of normal working hours (earlier or later than usual), please call or email the Assessor directly.
Who should be at the Assessment?
The Case Manager should invite any individuals with knowledge of the person served. This includes all paid service providers, the MCO, and the parent/guardian of the person, if applicable. The person for whom the assessment is being completed is required to be present at the meeting for at least part of the meeting. If it is not possible to meet in-person at the location of the team’s choice due to extenuating circumstances, we can utilize Microsoft Teams, however the person-served must be available via video. Otherwise, the assessment will be re-scheduled.
What documentation is needed?
Documentation is not required for the MFEI, but it helpful to have an updated physical and current medication list prior to the meeting. Assessors can use the information to prepare the health section ahead of time. This helps the meeting run as efficiently as possible.
